How to see our Therapists:
To see a physical therapist at South Mountain Physical Therapy, you can be referred by a healthcare provider, or opt for self-referral under Pennsylvania’s Direct Access Law.
Pennsylvania is one of many states that allow physical therapists to evaluate and treat, under certain conditions, patients without a referral from a physician for 30 days. After 30 days, you must obtain a physician’s referral to continue your treatment plan. Call your insurance plan before making an appointment to verify coverage.
Making an Appointment:
Call 610-927-5183 for our Sinking Spring office or 484-671-2065 for our Leesport office or stop by for an appointment. If it is after hours, please leave a message and we will get back to you. New patients can be seen within 24 hours.
Please:
- Bring your current insurance cards and photo ID
- Bring your prescription for physical therapy
- Arrive 15 minutes early for your first appointment to complete paperwork OR print and complete the intake paperwork prior to your first appointment.
- Wear appropriate clothing to allow access to painful areas
Initial Evaluations require 1 to 1 ½ hours, regular visits 45 min to 1 hour.
Handicap parking is available. We are wheelchair and ADA compliant.
Copayments are expected prior to every visit. We accept Visa, Discover, Mastercard, personal checks, and cash.
